Governing Law: State of California Please read these Terms carefully before booking or receiving services. By booking an appointment, purchasing a membership, or receiving services, you agree to these Terms.

1. ABOUT OUR SERVICES

IV League Mobile Concierge Hydration, operated by IV League Concierge Management LLC, provides mobile IV hydration and wellness therapy in Beverly Hills and Greater Los Angeles under the medical direction of Daniel Rossie, MD (CA License A157082). WELLNESS SERVICES ONLY: Our services are wellness treatments and are NOT medical treatment. They are NOT intended to diagnose, treat, cure, or prevent any disease, and do not replace the care of a licensed physician.

2. ELIGIBILITY‍ ‍

• You must be 18 years of age or older

• You must complete all intake forms before your appointment

• You must disclose all medications, supplements, conditions, and allergies

• You must not be pregnant unless cleared by your personal physician

• Treatment is subject to a clinical screening upon arrival and may be withheld if eligibility criteria are not met

3. APPOINTMENTS, TRAVEL FEE & CANCELLATION‍ ‍

• A non-refundable $50 travel fee applies to individual appointments and is due at booking

• Full payment for services is collected prior to or at the time of treatment

• Cancellations more than 2 hours before the appointment: drip payment refunded, travel fee retained

• Cancellations less than 2 hours before: drip payment refunded, travel fee retained

• No-shows: travel fee retained, drip payment refunded

• If IV League cancels: full refund including travel fee

4. MEMBERSHIPS & RECURRING BILLING‍ ‍

• Memberships are recurring subscriptions billed automatically on a monthly cycle to the payment method on file

• By purchasing a membership you authorize IV League Concierge Management LLC to charge your payment method automatically each billing cycle until cancelled

• Membership benefits (included drips, discounts, priority booking) are valid only while the membership is active and paid current

• Included drips do NOT roll over; unused drips in a billing cycle are forfeited at the end of that cycle

• Memberships may be cancelled at any time with 30 days written notice to hello@ivleaguela.com; cancellation takes effect at the end of the current paid billing cycle • No refunds are issued for the current billing month upon cancellation

• Founding Member rates, where offered, are locked for the duration of continuous membership but the membership itself remains cancellable

• We reserve the right to change membership pricing with 30 days notice; existing members will be notified before any change takes effect

• If an automatic payment fails, membership benefits may be suspended until payment is resolved

5. PAYMENTS‍ ‍

• Payments are processed through secure third-party processors

• You represent that you are authorized to use the payment method provided

• All prices are in U.S. dollars

6. NO GUARANTEE OF RESULTS‍ ‍

Individual results vary. No specific outcome, result, or health benefit is guaranteed. Any benefits experienced may be temporary.

7. LIMITATION OF LIABILITY‍ ‍

TO THE MAXIMUM EXTENT PERMITTED BY CALIFORNIA LAW, IV LEAGUE CONCIERGE MANAGEMENT LLC SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, OR CONSEQUENTIAL DAMAGES ARISING FROM OR RELATED TO OUR SERVICES. OUR TOTAL LIABILITY SHALL NOT EXCEED THE AMOUNT YOU PAID FOR THE SPECIFIC SERVICE GIVING RISE TO THE CLAIM.

8. ASSUMPTION OF RISK‍ ‍

IV therapy carries inherent risks including but not limited to bruising, swelling, pain, infection, infiltration, and allergic reaction. By receiving services you voluntarily assume these risks. Full risks are disclosed in your IV Treatment Consent, which you complete before treatment.

9. MEDIA RELEASE

Any use of your image or likeness for marketing is governed by a separate, optional Media Release that you may grant or decline without affecting your services.

10. GOVERNING LAW & DISPUTES‍ ‍

These Terms are governed by the laws of the State of California. Any dispute shall be resolved by binding arbitration in Los Angeles County, California. Before initiating any chargeback, you agree to contact us directly at hello@ivleaguela.com to resolve the matter.

11. CHANGES TO THESE TERMS‍ ‍

We may update these Terms from time to time. Continued use of our services or maintenance of an active membership after changes constitutes acceptance of the updated Terms.
